Our honest take
"No flush" niacin is inositol hexanicotinate: each capsule gives 800 mg of the compound, labelled as 640 mg niacin equivalents, about 40 times the daily reference intake. The compound releases only small amounts of free niacin, which is why it does not cause skin flushing, but that also means its effects are uncertain and it has not been shown to do what high-dose niacin was once prescribed for. If you simply want vitamin B3, a multivitamin covers it for pennies.
- You have a specific reason to take inositol hexanicotinate and have discussed it with your doctor
- You want to manage cholesterol or heart health: speak to your GP about proven treatments
- You have liver disease, diabetes or gout, or take statins or blood-thinning medicines
- You are pregnant or breastfeeding
The science
Niacin (nicotinic acid)· evidence: Limited
Niacin (vitamin B3) helps the body release energy from food. Adults need about 13 to 17 mg a day and most people get plenty from meat, fish, wholegrains, nuts and fortified foods, so deficiency is rare in the UK.
High-dose nicotinic acid (hundreds of milligrams to grams a day) used to be prescribed to change blood fats. Large trials found it did not reduce heart attacks or strokes in people already on statins, and it caused more side effects, so it is no longer used this way in the UK. Self-dosing at these levels is not advised.
Nicotinic acid causes "niacin flush": warmth, redness and itching of the skin, usually within 30 minutes. Sustained-release versions flush less but have been linked to more liver problems at high doses. Inositol hexanicotinate ("no flush niacin") releases little free niacin and its effects are uncertain.
Niacin contributes to normal energy-yielding metabolism. Niacin contributes to normal functioning of the nervous system. Niacin contributes to normal psychological function. Niacin contributes to the maintenance of normal mucous membranes. Niacin contributes to the maintenance of normal skin. Niacin contributes to the reduction of tiredness and fatigue. (Applies where a daily dose gives at least 15% of the NRV, i.e. 2.4 mg.)
Typical dose: Adults need about 13 mg (women) to 17 mg (men) a day (NRV on labels: 16 mg).
Safety: The UK Expert Group on Vitamins and Minerals set a guidance level of 17 mg a day of nicotinic acid from supplements, mainly to avoid flushing; EFSA's upper level is 10 mg a day. Doses of 500 mg or more, especially sustained-release forms, can cause liver damage, raised blood sugar, gout flares and stomach upset. Do not take high-dose niacin if you have liver disease, diabetes, gout or a stomach ulcer, or with statins, unless a doctor supervises it.
