Insufficient

evidence for betaine hcl

Our honest take

One capsule a day gives 650 mg of betaine HCl with 150 mg of pepsin, a combination sold to people who believe they have low stomach acid. There is no good evidence it improves digestion for most people, and the betaine dose is well below the 1.5 g a day linked to betaine's only authorised claim. It is cheap at about 10p a day, but if you have ongoing indigestion or reflux, see your GP first rather than adding acid.

  • You have discussed betaine HCl with a health professional and want a low-cost version
  • You want a single daily capsule taken with your main meal
  • You have stomach ulcers, gastritis or reflux, or take ibuprofen, aspirin or steroid tablets
  • You are vegetarian or vegan (gelatin capsule), or allergic to gluten, soy, milk or egg (may contain traces)
  • You are pregnant, breastfeeding or under 18
The science

Betaine HCl· evidence: Insufficient

Betaine hydrochloride is an acidic form of betaine, often paired with the enzyme pepsin and sold to people who think they have low stomach acid. A small study in healthy volunteers showed it can briefly lower stomach pH, but there is no good evidence that taking it improves digestion in people generally, and 'low stomach acid' is not something you can reliably judge from symptoms.

Betaine itself has one authorised claim, about homocysteine metabolism, but it needs 1.5 g of betaine a day. Typical betaine HCl capsules give much less than that and are not sold for that purpose.

If you have ongoing indigestion or reflux, see your GP rather than self-treating.

Authorised health claim
Betaine contributes to normal homocysteine metabolism. (Only for foods with at least 500 mg betaine per portion; the beneficial effect is obtained with a daily intake of 1.5 g of betaine. A daily intake above 4 g may significantly increase blood cholesterol levels.)

Typical dose: Betaine HCl products typically give 500 to 650 mg per capsule, taken with a protein-containing meal.

Safety: Do not take on an empty stomach. Avoid if you have stomach ulcers, gastritis or reflux, or take anti-inflammatory painkillers (NSAIDs such as ibuprofen or aspirin) or steroid tablets, as the acid can irritate the stomach lining. Stop if you get burning or pain. More than 4 g of betaine a day may raise blood cholesterol. Not for children, or in pregnancy or breastfeeding without medical advice.
