Our honest take
D-mannose, a simple sugar, as 500 mg capsules or as a pure powder. It is sold for urinary health, but a large UK trial in 2024 found it made no difference compared with placebo, there are no authorised claims, and we make no claims about urinary infections. If you have pain or burning when weeing, a fever or blood in your urine, see a pharmacist or GP rather than relying on this.
- You have discussed D-mannose with your GP and want to try it
- You want the choice of capsules or a pure powder (the powder is better value per gram)
- You have diabetes or take diabetes medicines, unless your GP or diabetes team agrees (it is a sugar)
- You have symptoms of an infection right now: see a pharmacist or GP
- You are pregnant or buying for a child, without medical advice
The science
D-mannose· evidence: Insufficient
D-mannose is a simple sugar related to glucose, found in small amounts in fruit such as cranberries. Most of it passes into the urine rather than being used for energy.
It is sold for urinary health. Early small trials were encouraging, but a large, well-run UK trial published in 2024 found it made no difference compared with placebo. There are no authorised health claims for D-mannose, so we make no claims about what it does. If you have pain or burning when weeing, a fever or blood in your urine, see a pharmacist or GP.
Typical dose: Studies have used 2 g a day.
Safety: It is a sugar: if you have diabetes, check with your GP or diabetes team first and monitor your blood glucose. High doses can cause bloating and loose stools. Not a substitute for antibiotics when they are needed. Ask your GP before use in pregnancy or for children.
